has still seeking me to be employed even with 6 doctors saying no and double

depression causing or reflecting a loss of an eye during this troublesome

time. Anyone know a good laywer?

10/5 I was informed that

I was terminated from Humana without written nor verbal warning. I lost my appointment and everything that I

had going. Leg, eye surgery, etc. I am in need of help anyone know a good

lawyer?

9-26 lost vision due to a operation on eye. Completely blind.

9/4 So yesterday the

Healthcare.gov made me cry. I called my Humana HMOx plan was informed I was not

a citizen of the US and that I needed to be verified and

was directly transferred to Healthcare.gov. Not what I had called in for. I

called in for a temporary prosthesis which Humana has denied, eye surgery codes

67370, 67028, j8035, 67114, a sleep study 95807 echo cardo 93306, dysprea anextion

986.05, referral for a Doctor Reid, and prescription for a Dr. Parkinson. After

20 minutes waiting for someone to pick up the phone, I hung up and called

Humana again where a nice lady listened to my requests for referrals. I will

give her a raise if I could but to no avail. Referrals not happening for me.

She to had to transfer me to Healthcare.gov 1 hour 25 minutes later, a Michelle

answered the phone. I was informed they could not find me. After 30 or 50

minutes of trying social, etc- I was asked to call Humana but to get a reason

as to why verification for a market place ID number. I called Humana- only to

find again that my social was not entered in, and they had no record of a

market place ID number, and that there was no government co-pay. The lady I

asked to stay on the phone with me from Humana. Fourth hour the guy picks up

and trys to find me. At least, he tried, phone, social, email, Humanas bogus

social entered into the system. They left me in tears fearing that I am now

having to identify myself to Market place department of health and human

service 465 industrial Blvd London ky 40750 saying I have to send in my social,

birth certificate, and families to prove I am an American. Case number from

human services- sorry they do not have one- from Humana 704073225914 so at

present, I also find out that HS is linked to IRS, Homeland security, etc.- Why

am I having to spend 5 hours on the phone to prove I am a resident not getting

subsidies per acknowledgment by Humana on the phone? What agency is looking at

me and why am I being punished, Mentally I am already crying I would like an

answer please. Then your collector called. I answered said I was Clinton and

blew up. I apologizes to her..

My concern is I have run out of money

from my withdrawal of IRA. I have had not income since Feb26. I am not wealthy and need some help. I need a

leg, a wheel chair, if possible my left eye back of a payment due to Humana HMO

waiting teterminate me from their health plan September 30 without written or

verbal notice. I was to have sleep studies, cardio photos and psychologist help

which I still need please.

The double depression was work related

in the sense that both jobs were stressful and both were asking me to do

something I did not believe was correct.I am applying for disability having lost left foot, right ankle, left eye sight, etc.

I spent three hours at credit union notarizing my licences, and social security card with all my family and coping my birth certificate. The next issue disability was supposed to make a point known August 13, per letters from Senator Hatch, and Honorable Matheson. That has been pushed to November 1st evidently I am to grow a leg, pay 29k versus 12k cap health bills. 10 4 I called to verify an appointment and was told verbally that I was terminated. I did not receive a letter requesting information. I did not receive a letter saying I was terminated.

I have 35000 worth of bills for a 12600 cap insurance. I have filed internal grievance with Humana, only to be denied.

I did not receive a wheelchair, nor prosthesis, and medically I am dealing meantally with some issues that I have no idea how to solve.

I lost my eye sight due to a surgery 9 26 from Doctor Teske which required another surgare. Due to financl situation and no social security I am now on Medicaid, however I am not scheduled currently for eye surgary due ot aointment etc.

Other issues’ they did not pay all bills and I am receiving more bills each day from different places.

March 3 amputation of left foot.

April 8 interview with social security.

PRomised in letter to Reresentative Matthewson determination to be made August 15.

Week of July 10 with told it was pushed to September.

In September they told me on medical hold till November.

I am still awaiting.
